HIPAA Notice

Basecare and our clinical partner, Altro Health, run separate systems. This page explains how your health information is divided between the two, and where your HIPAA rights apply.
Our Role in Your Health Information
Basecare Health Co. ("Basecare") operates the Basecare membership platform. Basecare is not a doctor's office, clinic, or pharmacy, and does not provide clinical care.
The clinical care available through your membership — intake, evaluation, diagnosis, treatment, and prescribing — is provided by Altro Health, a LegitScript-certified telehealth provider, and its network of independent, licensed clinicians. Altro Health, not Basecare, creates and maintains your medical records and, as a health care provider, is a "covered entity" under HIPAA (the Health Insurance Portability and Accountability Act).
Where Your Health Records Live
Health intake forms, appointments, visit notes, and clinician messaging happen inside the Altro Health portal, which is linked from — but separate from — your Basecare account. Altro Health's own Notice of Privacy Practices, provided when you set up your Altro Health portal, governs how your protected health information (PHI) is used and disclosed, and describes your HIPAA rights in full. If you can't locate it, contact your Altro Health clinician or Altro Health's portal support.
What Basecare Does Receive
To apply your correct member pricing and keep your membership and clinical accounts linked, Basecare shares a limited set of information with Altro Health — generally, that you're an active Basecare member and your membership tier — and Altro Health may confirm your enrollment status back to Basecare. This exchange does not include your diagnoses, treatment details, prescriptions, or other clinical information.
Your HIPAA Rights
As a patient of Altro Health's clinicians, you generally have the right to:
- Get a copy of your health and billing records.
- Correct your health and billing records.
- Request confidential communications.
- Ask Altro Health to limit what it shares.
- Get a list of certain disclosures of your information.
- Get a paper copy of Altro Health's privacy notice.
- Choose someone to act on your behalf.
- File a complaint if you believe your privacy rights were violated.
These rights are administered by Altro Health as the covered entity. To exercise them, use the Altro Health portal or contact Altro Health support directly.
Filing a Complaint
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your care.
